Please call us at 865.483.7415 to schedule your appointment. Once scheduled, we ask that do you the following before arriving for your first appointment:

• Logon to the Patient Portal and complete your medical history. ​​

  • You will be provided logon information by our office staff. ​

  • If you are unable to logon to the patient portal, please let our staff know so they can assist you or schedule adequate time to complete this prior to seeing your provider.

  • If not your medical history is not completed prior to your appointment, your appointment may be delayed or rescheduled to another day.

  • ​Complete and bring the Patient Information Packet​​ to your appointment​.

• Bring the following items to your appointment:

  • ID Card (State ID, Driver License, etc.)​

  • Heath Insurance Card(s)
